Deciding to put a loved one in a nursing home can be one of the toughest choices a family will face. Choosing a nursing home is not easy, either. Now the Centers for Medicare and Medicaid Services have released a database of 16,000 nursing homes nationwide.

The News and Observer reports that the data shows one out of every four nursing homes in North Carolina rates "much below average."
The nursing home rankings are based on three quality of care areas: health inspections; staffing; and quality measures.
