State officials say a Charlotte care facility where an autistic boy was staying before his death has serious safety problems.

The report released Monday comes about a week after a 10-year-old boy wandered away from the RHA Howell Care Center and drowned in a nearby creek.

State officials said the Clear Creek facility has two weeks to correct problems or it could be cut from the federal Medicaid program. North Carolina Department of Health and Human Services spokesman Jim Jones said the center also faces a $12,000 fine.

The same state agency previously said it was unaware of any "systemic" issues or violations involving RHA Howell facilities.

Federal rules bar the state from releasing the specifics of its report for 90 days. RHA Howell did not immediately return a call seeking comment.
