The U.S. Coast Guard echoes the NTSB in finding fault with management in the HMS Bounty sinking.
One crewmember died and the captain remains missing and is presumed dead after the tall sailing ship was lost off the coast of Cape Hatteras.
The Coast Guard says a combination of faulty management and crew risk assessment procedures contributed to the sinking.
The three-mast sailing ship was built for the 1962 film "Mutiny on the Bounty", and it sunk on October 29, 2012 as Hurricane Sandy moved up the east coast.
In February, the National transportation Safety Board blamed the sinking on the captain's "reckless decision" to sail into the path of the storm.
